What to Expect at Your First Visit
New Patient Paperwork
Before your first appointment, please print and fill out our New Patient Paperwork. Bring the completed forms with you to your first appointment, along with your current insurance card.
If you have not completed your forms, please arrive 15 minutes early to your appointment. We will have you complete the paperwork when you arrive for your appointment. Please bring your current insurance card.
Patient History
Dr. Martini will discuss the nature and extent of your condition with you to determine if chiropractic care is an appropriate treatment for the condition. If chiropractic care is not suitable for your condition, we readily refer patients to the appropriate health care provider.
Physical Exam
Orthopedic, neurological and spinal examination as well as posture analysis, are performed to determine general health status and specific problems.
X-rays
X-rays may be needed to further assess the health condition if necessary.
